Fingerprints and the Child Abuse Index Check form. |

A Health Screening Form dated no earlier than four months prior to the date of |

application, completed by a licensed physician or nurse practitioner. |

Documentation of a TB test with a negative result taken no earlier than four months prior |

to the date of application submission. |

Transcripts of all college-level Early Childhood Education classes. |

Social Security Card or birth certificate and California state identification card, either |

Driver’s License or Identity Card. |

Current First Aid / CPR / Health and Safety Certification |
